• Low Income Individuals and Families.
• Total household income must be at or below 250% of the federal poverty guidelines

• Complete ECCO Client Eligibility Form available on website. All documentation must be submitted through this form before an appointment can be scheduled.

Documents

• Proof of Identity
• Copy of current Photo ID such as a driver's license or state issued Photo ID.
• Proof of income
• Pay stubs for the last 30 days or award letter for public assistance, social security, or unemployment.
• If self-employed: complete self-declaration of income form on website
• If performing an undocumented job: complete statement of income from employer form on website.
• Proof of Residency
• Most recent utility bill or mortgage agreement/lease agreement.
